The Pharmacotherapy Specialist is a clinical pharmacist with advanced expertise in managing complex medication therapies for hospitalized and clinic patients who cross the continuum of care. This role focuses on optimizing pharmacologic treatment plans in collaboration with multidisciplinary teams to ensure safe, effective, and evidence-based care across various inpatient units and crosses care to patient in the outpatient clinical space. Qualifications

  • Doctorate Degree Pharmacy
  • License/Registration - Issued by Florida or Compact State - Pharmacist
  • Pharmacy degree (Pharm.D) from an accredited college of pharmacy.
  • Completion of a PGY2 pharmacy residency accredited by ASHP in relevant environment, or equivalent work experience in specialty area, required.
  • At least 5 years of clinical practice in a hospital setting.
  • Residency training contributes towards this requirement.
