Suncoast Hospice, a part of Empath Health is seeking a Patient Concierge (Care Navigator) to join our team. A first responder from our agency who is passionate about the services we offer, and works directly with patient/family/authorized representative, or any other customer to insure optimal access. Keeps patient centered services and caring at the forefront, exceeding customer expectations and insuring best practice and flawless care in all aspects.
Schedule: Monday-Friday 9am-6pm

What you'll Do
- Quality: Completes required documentation in real time by being accurate, thorough and detail-oriented from referral through admission.
- Ability to work alongside the interdisciplinary team in all settings, while traveling to where the patient is.
- Promotes social and emotional well being: Able to assist with inquiries/education regarding self-determined life closure issues (Advance Directives), while also being knowledgeable about FL Proxy Guidelines to insure proper signage of all forms related to admission process.
- Stewardship: Continually acts as an ambassador for Empath Health when communicating with all customers.
- Teamwork: Participates and promotes the agency strategic goals related specific to access, customer service and quality
- Professional Development: Consistently updates knowledge of Empath Health programs

What You'll Need
- Education and/or Experience:High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
- Excellent verbal and written communication skills; knowledge of medical terminology.
- Computer skills.
- Knowledge of Empath Health Hospice programs and of community resources.
- Basic knowledge of reimbursement for services.
- Employees must have reliable transportation/insurance and be willing to travel throughout the county which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee's HR file.
